When I play correspondence chess here on lichess, sometimes people will pre-move.
Pre-move is a valid tool for blitz chess, but doing this on correspondence is just rude.
I always block people who do that.

I'm not a fan of the whole shaking hands before a game for two reasons:
1) If you have to do it to play a game without forfeiting, you're not shaking hands out of respect for your opponent but because you have to in order for the game to officially start.
2) If I was playing someone who was talking shit about me the day/week/month before, why should I be expected to shake his hand?"
Because for this game, you set aside your personal negative feelings and play sport. Original poster called it etiquette, but theres another word for it - sportsmanship.

#2 your second point is interesting, but I view it from a different standpoint:
The handshake before the game is the agreement to a respectful fight for the win. Refusing to shake someone's hand before the game essentially says that you are not able to put aside personal stuff to play this game with respect to your opponent, so you shouldn't play the game at all. This is why I think FIDE's rule of losing the game for refusing handshakes is agreeable.
Now there are other reasons you might not like shaking someone's hand, for example you have a hygienic tic and just can't. That's forgiveable ofcourse, and it does mean you are still willing to give respect to your opponent, but refusing because you don't like your opponent is unacceptable to me.

@Seallv So you're telling me that if I was to say something along the lines of "Seallv is the biggest dickhead you will ever meet, I hope he dies in a car crash" (obviously I don't mean that, it's just an example) minutes before we were to play in a tournament, you'd really shake my hand? Because I wouldn't. The only thing I'd do with my hand in that case is punching whoever said it.
Pre moving in correspondence is a built in feature called analysis board. This helps speed up the game by playing theory or forced moves and not have to way for days or weeks to make them. This is not rude, it is courteous and you should rethink blocking people who pre-move. Everyone pre-moves.
As for handshaking, this is an outdated custom that is highly unintelligent due to the spread of germs as well as having to make physical contact with strangers. The Asians had it right with Bowing. Bowing shows respect minus stupidity.
