Years of reports with no indication that any reported user facing any consequences.

This is my experience now. Please explain how this could be true. Lets say there are 500 reports over 2 years. How could this realistically be the case. What am I missing.

I have at least twice received emails from Lichess telling me that my report has led to a ban.

If this were the case, you reported approximately every opponent. Perhaps your instincts aren't as good as you think.

reporting for what? Asusumin fair play two explanation pop up

  • you reported player who are not violating
  • almost never one game is sufficient evidence i.e. your report was handled but action was taken so much later that you did not receive any ratings point compensation hence no notification

I would endeavour to guess that combination of the two.

A century ago, Nimzowitsch famously shouted 'How can I lose to this idiot?' after a defeat.
It's that sense of entitlement, you are stronger than your opponent, understand chess better and work harder at it.
The modern version: 'How can I lose to this cheating idiot?' They're definitely an idiot and it's impossible they could win without assistance.
