Every single time. I've never seen someone being correct in a post complaining about a puzzle.

Bxf7+ is not mate. Kf8, you lost the checkmate sequence and the game is equal. Qxf7+ leads to checkmate on the next move. When you are done with the puzzle turn on the engine and try the moves you think are correct to figure out why you're wrong.

As i-bex said Kf8 allows the king to escape
As for the puzzle just use

and then add to the string the puzzle number without the hash
