Not all! Some positions, for example if you start at a losing position, and then get an equal position by a good combination are also important. Puzzles should only be deleted if:
Start equal and end equal
Start losing and end losing
Start winning and end winning
So when the combination doesnt change the result.
Or if the combination itself is false somehow.
Also, most wrong puzzles get deleted very fast, because many people downvote it and lichess takes it down. So i dont get your problem

I have seen many great puzzles that get downvoted a lot only because they are much more complicated than they seem at first and take 30min to calculate properly. I think bad puzzles do exist, but are very very very rare. Why don't you show some examples, so we can look at them one at a time?
The only minor issue that I know of is that certain types of puzzles tend to be over- or underrated. For example, if you are in check and have only two legal moves, sometimes you have to see at least 6 moves ahead to understand why one move doesn't win, but people just guess the right move and the rating gets surprisingly low. But still, I really like lichess puzzles.

Maybe they should rethink the entire scoring system. People downvote when they fail and that's it. Most of the best puzzles I've seen have very low scores. A pity.

If the puzzle has 0 scores, then the puzzle is really incorrect. It's not about that people downvote a puzzle, because it was too complicate. Of course there are such of them, but probably with -10, -20 downvote scores. But if many many people downvote to ZERO, it has a reason.
