Paired with the same opponent twice

I played twice against the same opponent.This should not actually happen with the Swiss system! See first and last round.

It's happened to me before.

In this swiss tournament it paired 2 pair of opponents twice. and it did not follow the pairing rules .

This happened on Wednesday also in our tournament: lichess.org/swiss/BPryqbLj See, for example, the tournament winner, who played twice against two (!) players. We have had Swiss tournaments at least twice a week for several months and, as far as I know, this has not happened previously.

#3 I'm trying to figure out what happened in this simple example... even the first-round pairings surprise me, since I would expect pairings (by rating):
1 vs 7
2 vs 8
etc.
6 vs 12

I do appreciate the examples however I'm kind of losing my mind trying to understand where the errors lie:
a) pairings engine
b) communication between lila and pairings engine
c) lila

Presently it is possible (although there are no guarantees in the future that it will remain possible) to download the TRF file by adding ".trf" to the tournament URL. Given this file, any pairing software should always generate the same Swiss (Dutch) pairings.

I'm pretty sure that is English... if I knew what the answer was or knew any simpler way to describe it, I would have done so; but I am stumped.

I think the issue have been resolved? I pulled a couple of the current tournaments into Swiss Manager and it did not kick out complaints about double pairing.
Can anyone confirm?
