So, French defense players will most likely go for the main line, or the exchange.

But has anyone ever in-depth mainly used the Mediterranean as a reply for the standard 1. e4 e6 2. d4 ___ ?

This 1 e4 e6 2 d4 Nf6 looks like a bad version of Alekhine's Defence, as the move 1...e6 hinders Bc8 from developing to g4 or f4.
