It can be misclick but base on my inference (with numbers)

Base on the observations above
1.) Offering a draw agains't a master will annoy him, especially at a rated game.
2.) It is a rated game, so it is unlikely for everyone to forgive misclicks.

3.) It is pretty annoying to see this, even if you are not a master.
4.) 1-2 takeback sent is enough, if you think your opponent wont forgive misclick's then play on or resign (this is what I do)

(I have a broken mouse that sometimes double click, but I can use it to move my cursor quickly then use my mousepad to click.)
