Because you opened lines that leads to attacks. Maybe you can defend it but I saw that you moved wrong. So it leads to lose. You can defend it properly with Be7 to defend f6.

As @ericmsd already noted, you seem to be unaware of how to use and/or interpret the engine analysis. It tells you that not only 16. Qxh6?? was a blunder, it lists 3 blunders, 2 mistakes, and 6 inaccuracies in total.
It is possible that not all of the moves the engine lists as inaccuracies with the low depth of this automatic analysis actually are inaccuracies, but the 3 blunders definitely were serious mistakes you made. The other two are 35. c3?? and 38. Re1+??.
