Why do I play so bad?

The answer is... Why am i doing this next move? :) For example the game on top you capture 12. Qxe4 ?! 1. why offering the Queensexchange? 2. Why is it better to capture with the bishop? You re in a complete won middle game there if 12. Bxe4 !
Why? 1. Your Opponent did not castle short and will not be able to do so for the next 4 moves... Time to create some pressure!
Time to bring on the rook in the e file and raise the pressure. At the same time you have pressure on the diagonal and Bx c6 is threatening ...the knight must move to e5. Q to the e file rok to the e file and the e file is so overload that it is a question of a few moves and you have your opponent crushed. But the reason you lost is that you did not know why you did capture with the Q and let him do the Queensexchange...and all the before created pressure is gone. You played great till 11. and 12. So try to learn to know why :)
Thanks to all for good advice.

@HORRORMAKER : "You played great till..." This happens sometimes. Chess of chance less or more.
you already answered your own question: you don't practice.

so practice.
Playing bullet/blitz will help you to find tricks you fall into and then you can watch what you could have done. Don't just copy engine moves, but look what stronger players played there and try to understand why. Bullet is a good way to see multiple types of positions in a short time. You will remember them better, because you played them by yourself. Of course you can read theory from the books, if you are interested, but I think you should have played chess some time before you do that.
